Immunization Information for Students Entering Grade 6

Dear Parents, Guardians, and Students,

The Indiana State Department of Health requires the Tdap (pertussis) vaccine, Menactra (meningococcal) vaccine and 2 doses of the varicella (chicken pox) vaccine (unless we have written documentation that your child has a history of the chicken pox disease) for students in grade 6. This is a legal requirement that all students entering grades 6-12 will need to have a record of these immunizations.

The immunization record needs to be on file in the school nurse's office within 20 calendar days of the beginning of the school year. If your child does not have records on file within the first 20 days of the beginning of the school year they will be excluded from school until those records are received. 

Below are the requirements for students in grades 6 to 12:

Grades 6 to 11 3 Hep B
5 DTaP
4 Polio
2 MMR
2 Hep A
2 Varicella
1 Tdap (Tetanus & Pertussis)
1 MCV4 (Meningococcal conjugate)


Grade 12 3 Hep B
5 DTap
4 Polio
2 MMR
2 Varicella
1 Tdap
2 MCV4
2 Hep A
